Carmel Manor has completed our expansion and renovation! We need more staff to work in our brand new facilities!

Under the direction of the Charge Nurse, assists in providing professional nursing care to residents as determined by qualifications and training.

DUTIES AND RESPONSIBILITIES:
Performs assigned nursing procedures for the comfort and well-being of residents by maintaining philosophy of care and nursing standards within assigned unit.
Performs professional duties as determined by qualifications and training such as administering medications and carrying out treatment orders.
Reports observations of residents conditions to supervisor.
Maintains adequate, accurate clinical records and completes required forms if resident is being transferred.
Provides treatment using therapeutic equipment.
Takes temperature, pulse, respiration rate, blood pressure and heart rate to detect deviations from normal.
Records nursing needs of residents on nursing care plan to assure continuity of care.
Provides clinical supervision of nursing assistants providing direct resident care.
Counsels nurse aide and recommends disciplinary action, when necessary, to the Director of Nursing.
Prepares nurse aide assignments and explains pertinent resident care.
Notifies physician, when necessary, of changes in residents conditions, weight, medication errors, medication not available, abnormal x-rays/laboratory results and any other unusual observations.
Participates in resident care conference as scheduled.
Conducts resident rounds daily, reporting problems to nursing supervisor and initiating corrective action.
Identifies resident problems and emergency situations, and initiates immediate “life-saving” measures.
Follows staffing guidelines and makes assignments according to qualifications of staff and characteristics of resident population.
Performs and documents comprehensive assessment of resident.
Knowledgeably and competently preforms basic nursing skills.
Follows facility discharge/transfer procedures and coordinates discharge/transfer with other departments, physicians and ancillary service providers. Completes documentation related to resident’s discharge/transfer.
Supervises response to residents calls for assistance.
Counts narcotics, checks emergency kit and exchanges keys with oncoming nurses.
Provides report to oncoming nurse, including sufficient information for follow-up action if necessary.
Identifies hazards and initiates corrective action immediately.
Interprets existing policies and procedures to nurse aides, restorative nurse aides, residents, families and physicians.
Documents resident care provided and resident’s response or lack of response to care provided.
Follows facility admission procedure and communicates resident admission to other departments, physicians and ancillary service providers.
Assesses resident needs and initiates development of individualized plans of resident care.
Reviews and revises resident care plans as necessary and as required by regulations.
Listens to resident, family, physician or staff concerns and initiates corrective action and/or reports concerns to Nursing Supervisor.
Orders medication as necessary.
Observes, evaluates and documents all changes in resident’s conditions.
Documents progress notes as assigned.
Documents daily notes on unstable residents, Medicare residents and recently admitted residents.
Observes infection control procedures.
Participates in the employee evaluation process of nursing assistants.
Supervises and evaluates all direct resident care and initiates corrective action as necessary.
Requisitions supplies, equipment and linens to meet unit needs.
Notifies supervisor when residents condition changes or resident becomes critically ill.
May be asked to rotate among various clinical units of the facility.
Continues professional growth through in-service meetings, current literature, institutes and workshops.
Attends department meetings.
Advises nursing survivor of Director of Nursing of topics for discussion at staff meetings.
Supports and participates in facility Quality Assurance program.

EDUCATION AND EXPERIENCE:
Graduation from an accredited nurse program with current licensure by Kentucky State Board of Nursing. Six months to one year experience in medical-surgical nursing preferred. Verbal and written communication skills necessary to understand instructions and to communicate with residents and staff. (English language required.)

PHYSICAL DEMANDS:
Work is heavy. Standing and walking most of the time on duty. Lifting and transferring residents, carts, wheelchairs and equipment. Reaching, handling and fingering equipment and supplies. Stooping and bending while administering care. Audio acuity to make fine discrimination's in sound. Visual acuity for reading of equipment.
